You should be contacted from Dr. Love’s office, after you have made an appointment, and sent a Restore Urology New Patient Registration Form. You can fill this form out on-line, on a phone, tablet, laptop or desk computer, and easily return it to us by clicking the ‘Submit” button. If you don’t receive that registration form, there is a link to it below, so you can print it, complete the form and bring it with you on the day of your appointment.
To help you assess and keep a track of your symptoms and medical problems, Dr. love requests that you fill out questionnaires related to your problem. You will receive a pre-consultation phone consultation from our Nurse Practitioner, who will make sure you and we have all the information needed for an efficient consultation, and the Nurse Practitioner will send you the appropriate questionnaire for your problem after that phone consultation.
