How do Penile Implants Work?

There are two different penile implants to consider – an inflatable implant or a flexible implant:

Inflatable Implant

An inflatable penile implant is the best and most popular ,and gives a more natural erection, very similar to your own erection before you developed ED.

  • There are three parts to the inflatable implant, which are all connected by silicone tubings:
  • A reservoir (placed in the abdomen) where the fluid is located when the penis is flaccid
  • Two cylinders (placed in the shaft of the penis) which fill with fluid to give the erection
  • A pump (placed in the scrotum between the testicles) which transfers the fluid from the reservoir to the cylinders


When you squeeze the pump, fluid (usually sterile normal saline) is moved from the reservoir into the penile cylinders, creating an erection. When you press the deflate button on the pump, the fluid moves out of the penis and back into the reservoir for a natural looking flaccid penis”

Malleable/Flexible Implant

A malleable penile implant consists of two firm but flexible rods that are placed into the shaft of the penis.

There are no other parts to this implant, so the device is simpler. To have an erection, you simply hold the penis and move it into the desired position. When you are finished, you bend it back down, or to the side, to conceal it. This implant suits those with limited dexterity.

These implants give good rigidity to the penis, but do not affect the width or length of the penis, and it cannot be made flaccid, so the penis is always “erect”, just bend away to conceal the device.

What is the surgical procedure for a Penile Implant?

Dr Love uses the MINT (Minimally Invasive No Touch) Technique which he developed. This technique has resulted in some of the world’s lowest infection rates for Penile Implant Surgery. 

Penile Implant surgery is performed under sterile conditions in the operating room with the patient under general or spinal anaesthesia. The operation usually takes less than 1 hour with a single overnight stay after surgery in the hospital.

An inflatable penile implant offers an on-demand erection in just 15 20-30 seconds — whenever you want, for however long you want it. They are only erect once pumped up, and the penis is flaccid when the implant is deflated. 

Malleable, or semi-rigid, implants are hard all the time, but can be bent in the direction the man wants for sexual activity, or to conceal the implant.

Yes, penile implants can allow for sexual pleasure and sensation during intercourse. 

An implant gives the penis the hardness that is needed for penetrative sexual activity, but other aspects of sexual function should not be changed with an implant.

While the sensation may vary a little from natural erections, most men report satisfactory sexual experiences with penile implants.

While penile implants can restore erectile function for men with erectile dysfunction, there are potential downsides to consider. These may include surgical risks, such as infection or mechanical failure of the implant. There is no “natural” erection with an implant in the penis, although for most men having an implant they are experiencing this problem anyway.

Additionally, a very small number of men may experience discomfort or changes in sensation following implantation.

Inflatable penile implants are designed to be discreet, and provide a natural-looking appearance when the penis is flaccid. However, when inflated to produce an erection, the penis may appear even fuller and firmer than during a natural erection. Overall, the goal of penile implant surgery is to restore erectile function while maintaining a natural appearance as much as possible.
